Summit Family Practice Anchorage will be permanently closed as of the end of the business day on December 16th, 2016.
Dr. Hornbein will begin working at Medical Park Family Care on December 19th.
To assist in Dr. Hornbein’s transition, the Anchorage office will be limiting appointments between November 28th and December 16th. Dr. Hornbein’s schedule at Medical Park Family Care will open after January 1, 2017.
Medical Park Family Care welcomes ALL established patients of Summit Family Practice, including those with Medicare and Medicaid.
For SFP patients who have Medicare/Medicaid and who wish to become patients of Medical Park Family Care, you must see Dr. Hornbein at Medical Park for a first visit, after which you can chose any of the Medical Park Family Care providers to be your primary provider. If you need to be seen for an urgent medical problem between November 28th and December 16th:
If you are insured by Medicare or Medicaid, call SFP at 272-3366.
All others, call Medical Park Family Care at 279-8486.
FOR MEDICAL RECORDS BEFORE DECEMBER 16TH
If you choose to transfer your medical care to another facility, you will need to complete “Authorization to Release” form, we have one located on our website and return it to Summit Family no later than December 16th.
